How to write a formal request for data?

I have a request to write data to business customers (I work in a business management consultant and the client need an official document, rather than a simple email list.) Is there a standard format for writing data queries? If not, there are models? If not, there are elements that are absolutely necessary (eg, What are the data I need, I'll use it for)? Thank you in advance

You write a standard letter. When you arrive at the section on application data, includes a paragraph, the following: "To do this work, we need some information about your organization. Elements we have selected are similar in scope to what we normally receive from our other clients working in this nature, and are appropriate for further analysis. Please provide: a) b) c) …. As work progresses, it may request additional information. We will our demands to a minimum, but consistent with the implementation of a thorough analysis. If any of the data elements are required task not available or are too difficult to make, please call us to discuss practical alternatives.
